This lesion does not involve the eyelid margin, but large lesions like this one are a challenge for reconstruction because of the chance of lower eyelid ectropion. B. Notching of the eyelid margin is a sign of an eyelid neoplasm. This basal cell carcinoma has caused a notch and demonstrates the smooth pearly borders of a basal cell carcinoma.
