Figure 16-11 || Right shoulder external (A and C) and 70-degree arthroscopic (B and D) views demonstrate how an accessory anterior portal often is required to gain the appropriate angle to the lesser tuberosity ( green arrows ). This angle often results in instruments being used very close to the patient’s face (C) because of the retroversion of the proximal humerus. Depending on the patient’s anatomy, an ASL portal ( red arrow ) also can provide a good angle of approach for upper subscapularis anchor placement with rotation of the humerus.
