CHAPTER 3 | Instability
FIGURE 3-99 Right shoulder, anterosuperolateral viewing portal. The anterior capsular attachments to the humerus can be seen down to 5 o’clock, and they are intact down to that level. A, anterior capsule; G, glenoid; H, humeral head.
FIGURE 3-97 MRI. Coronal cut suggests a HAGL lesion.
Arthroscopic Findings: ■ Inferior capsular attachments to the humerus were attenuated and torn (Fig. 3-98). ■ Anterior capsular attachments to the humerus could be seen down to 5 o’clock without disruption (Fig. 3-99).
■ After debridement of torn capsular margins, the HAGL was seen to extend from 5:30 to 6:30, in the farthest inferior portion of the shoulder. A posteroinferior (7 o’clock) working portal was established for bone bed preparation and repair (Fig. 3-100).
FIGURE 3-100 Right shoulder, posterior viewing portal. A posteroinferior (7 o’clock) working portal has been established, and a ring curette has been introduced through that portal for bone bed preparation. H, humeral head; P, Posteroinferior capsule.
FIGURE 3-98 Right shoulder, posterior viewing portal. The humeral attachments of the inferior capsule from 5:30 to 6:30 are disrupted. H, humeral head.
